Affordable Care Act (ACA) Reporting
Mattex Service Company, Inc. is required to report information to the IRS regarding the health insurance coverage offered to full‑time employees. This information is reported on Form 1095‑C.
Form 1095‑C Availability
Beginning with the 2024 tax year, employers are permitted to provide Form 1095‑C upon request rather than mailing the form automatically. Mattex Service Company, Inc. will not automatically mail 2025 Form 1095‑C to all employees. If you need a copy, we are happy to provide one upon request.
Is Form 1095‑C Required for Filing Taxes?
No. Form 1095‑C is not required to complete your federal tax return. Your tax preparer may ask for the form, but you can simply provide the months during which you had health insurance coverage.
